Updated documentation for 1.4 release

At Dims's request, I've updated the documentation distributed with Axis for the upcoming 1.4 release. I've changed it to reflect the current version number, as well as updated the changelog and svn logs. I've also removed all references to the old CVS repository and changed the docs to reflect the use of svn. It would be helpful if there were a page similar to http://ws.apache.org/axis/cvs.html for SVN to point people at in the docs and to link off the website. I'm happy to write up the text for such a page and submit it if that would be helpful – let me know. It might also be good to create a versions field for 1.4 in JIRA at some point.
