Rename AXIS2_OP_GET_PHASES_* to AXIS2_OP_GET_*

Consider the case AXIS2_OP_GET_PHASES_OUTFLOW. Here what we get is an array list of outflow. So the name is kind of misleading and I see that java implementation has renamed it to AXIS2_OP_GET_OUTFLOW - which is apropriate.
