Support CREATE TABLE in tests (and only in tests)

Support CREATE TABLE in tests. It is useful to be able to create tables with small amounts of test data for the duration of tests. We should add CREATE TABLE the same way that we added ALTER SYSTEM UPLOAD JAR in CALCITE-1384.

Also, make it available to Quidem tests via a connection specification and schema called BLANK.
