Correct example/csv/TUTORIAL.md according to the current package names

example/csv/TUTORIAL.md references non-existing classes after package renaming.

The tutorial needs to be corrected and Optiq references should be chaged to Calcite (e.g. optiq-csv -> calcite-csv)
