Don't use implicit ManageDsaIT control magic of JNDI

JNDI automatically adds the ManageDsaIT control if environment property Context.REFERRAL is set to "ignore". In some code we use this "magic". Instead we should always set Context.REFERRAL to "throw" and set the control manually if needed.
