Duplicate registration of ConfigurationAdmin from PersistenceManagerTracker

When custom instances of org.apache.felix.cm.PersistenceManager are unregistered and registered quickly, there's certain chance that such instance will be used twice to activate the Configuration Admin.

I'll send a PR which protects from using the same PersistanceManager more than once.
