Improve documentation of namespace handling

The namespace handling deserves some more documentation with regards to:

• differences between namespace declaration in CND files and XML namespace declaration
• which prefixes to use in JCR paths and values
• what potential issues arise with conflicting namespace declaration in package and target repository
