JDBCKnoxShellTableBuilder should have optional username and password fields

During splitting up the old KnoxShell table into meaningful classes (see KNOX-2022) the username/password fields were mistakenly left out from JDBCKnoxShellTableBuilder.

Important note: the call history framework should not display raw passwords.
