Deliver slaveLost / executorLost reliably

The slaveLost and executorLost callbacks are not delivered reliably. This makes it hard for frameworks to rely on these features. We should consider making these signals reliable, e.g., by extending the concept of reconciliation to support reconciling the status of slaves and executors.
