Perform additional scan in PhoenixTransactionalIndexer.getIndexUpdates() only if required

Use the uncommittedPhysicalNames information in MutationState to detect if we have written to a table and are doing a rollback and only then perform a second scan in PhoenixTransactionalIndexer.getIndexUpdates().
