header_rewrite uses TSUrlHostSet() when using set-destination PATH

I noticed when using set-destination PATH <value> that the origin host would be <value> in the logs. After looking through operators.cc, seems like there was a bad copy/paste where case URL_QUAL_PATH should have used TSUrlPathSet() instead of TSUrlHostSet().
